② Certificate of Registered Information on Birth Registration: Authenticated copy of the birth registration issued by the local government office （Japan) (original + 4 copies)
③ English translation of the certificate of registered information on the birth registration form: original + 4 copies
④ Birth certificate for Filipino nationals: Original + 4 copies
⑤ Passports for both husband and wife
⑥ Letter Pack Plus: 600 yen (as of May 2026)
Regarding item ③, an English translation of the certificate of registered information for the birth registration is acceptable if you can prepare it.
You can get this translated for 3,750 yen.
Although we couldn’t find any information on the embassy’s website or elsewhere stating that an English translation was required beforehand,
we paid 3,750 yen to have it translated for us because it was too much trouble for us.
